• Hard hats & proper footwear are required at all times.
• Smoking is strictly PROHIBITED.
• Hot work permit required daily for any/all hot work.
• Proper PPE is required when conditions/tasks dictate – i.e. safety glasses/face shields.
• Each subcontractor is responsible to maintain a clean & safe work area.
• Contact Harvey Construction of any/all accidents or injuries.
• Contact Harvey Construction of any/all chemical spills.
• Under no circumstance is live electrical work permitted.
• Workers are required to park at designated areas ONLY.
• GFCI protection is required at all times. Any defective cords will be cut & removed.
• OSHA standards are to be adhered to at all times. NOTE: FAILURE TO COMPLY W/THESE SAFETY RULES WILL SUBJECT WORKER TO FINE AND/OR EXPULSION FROM JOB SITE

Each and every worker who enters this work site is required to view the Harvey Construction Corporation Safety Orientation video on the next page.

Upon viewing and understanding these guidelines, each worker will receive a site specific “Safety 1st” sticker. This sticker is to be placed on the individual’s hard hat, which will allow Harvey Construction staff to identify all persons who are in compliance.

I also understand and agree to abide by the Site-Specific elements of the project as presented by the Superintendent. I agree that I have access to safety programs & SDS sheets for any hazardous substance that I may use or encounter. I understand that if I do not have proper training or any other specific safety training that it is my responsibility to notify my supervisor immediately.
